For the Helios Frankenwaldklinik in Kronach we are looking for a part-time social service employee (m/f/d) with immediate effect.

• Advice, organisation and coordination of post-hospital care as well as communication with further treatment facilities
• Transition management for patients in need of care, including research and mediation of suitable care places
• Implementation and support of patient-related assessments for needs assessment and care planning
• Advice and support for patients and relatives on administrative and social law issues
• Support in obtaining cost commitments for rehabilitative measures and close interdisciplinary cooperation with medical services and nursing
• Coordination of the supply of remedies and aids for further care at home as well as training of patients and relatives

• Completed studies in social pedagogy or social work or qualified medical vocational training (e.g. health and nursing, nurse, medical assistant (m/f/d) or comparable qualification)
• Sound knowledge of the legal foundations of the Social Security Code, in particular SGB IX and SGB XI
• Confident and routine handling of official documents and administrative procedures
• Experienced handling of PC and MS Office applications
• Application-safe use of iMedOne functions in a professional context
• Proof of existing measles protection in accordance with the legal requirements
