Employee Social Service / Social Worker (m/f/d)

We are looking for you as soon as possible at the Helios Clinic in Leisnig, initially for a limited period as part of a parental leave replacement until 2028, as an employee in social services / social workers (m/f/d).

What to expect

Planning, coordination and further development of discharge management
Networking with external providers of care, care and rehabilitation
Service-conscious and humanly committed advice and care for patients, relatives and payers
Advice and information on possible social and insurance law assistance in the field of social security, long-term care insurance, care law and financial support options according to the Social Security Code
Initiation of rehabilitation
Support in the application process, commissioning of outpatient care services and referral to inpatient care facilities

What you bring to the table

Degree in social work/social pedagogy or comparable qualification
Industry-specific professional experience, extensive knowledge of social security law and willingness to continuously update
Very good, patient-oriented conversation skills
Ability to control tasks, forward-looking thinking/acting, high empathy and sense of responsibility
Very good time management, independent work with a solution-oriented approach, serious appearance and absolute discretion
Proof of existing measles protection in accordance with the legal requirements
