We are looking for you at the Leipzig Heart Center as soon as possible as

Heart Failure Nurse / Nurse / Nurse / Nurse / MFA / Medical Assistant (m/f/d)

full-time or part-time.

• You will be responsible for coordinating care plans for patients with heart failure in close cooperation with doctors and nursing staff
• Your area of responsibility will include the organization and implementation of training courses for patients and their families
• You will be responsible for monitoring adherence to treatment plans and for medication management
• You will be responsible for documenting and reporting on the progress of patients
• You will take part in interdisciplinary team meetings and make an active contribution to the further development of the treatment processes
• You will be responsible for developing and implementing strategies to improve patient care

• You have completed vocational training in nursing or a related field as well as several years of experience in caring for heart failure patients or in cardiology are an advantage
• You have the additional qualification of heart failure nurse / heart failure assistant or are willing to complete this advanced training
• You have excellent communication skills and a high degree of empathy
• You will have the ability to collaborate in a dynamic, interdisciplinary team and have in-depth knowledge of managing and documenting care plans
• Your strengths include attention to detail and excellent organizational skills
• Legally required proof of immunization against measles
