To strengthen our practice team in the Medical Care Center at the Bad Saarow site, we are looking for you as soon as possible as

Diabetes Consultant (m/f/d)

full-time or part-time (at least 16 hours / week)

• You will be responsible for coordinating our diabetological consultation hours
• After consultation with the attending physician, they independently conduct and evaluate individualized consultations and training courses in compliance with current guidelines and evidence-based data
• The area of responsibility includes the implementation of structured training courses in the field of disease management programs for patients with diabetes mellitus type 1 and 2
• They accompany the adequate implementation of the medically prescribed therapy and promote the self-management of the patients
• You will also support the selection of remedies and aids for the care of people with diabetes mellitus
• They guide people with diabetes mellitus or their relatives in documenting the therapeutic measures and check them

• You have successfully completed further training as a diabetes consultant at the DDG (m/f/d), preferably also have a medical training background
• Preferably, you already have previous experience with structured training modules and AID systems
• You are no stranger to a willingness to take responsibility, empathy and the ability to work in a team in the spirit of empowerment
• You also have an open and friendly demeanor, commitment and enthusiasm
• Legally required proof of immunization against measles
