We are looking for you as a social worker (m/f/d) at the Helios Clinic in Hettstedt as soon as possible .

What to expect

Empathetic support of patients and relatives through challenging life situations as well as individual advice on social, financial or legal issues
Coordination of offers of help, support in applying for social benefits and referral to external specialist agencies
Review and processing of cost assumptions by social welfare agencies as well as clarification of inpatient treatment costs for patients without health insurance
Organisation and coordination of post-hospital care, including the initiation of rehabilitation measures and referral to suitable institutions
Communication and coordination with referring physicians, cost bearers as well as rehabilitation facilities or other aftercare facilities
Active contribution to promoting the psychosocial stability of our patients and positively influencing the length of stay

What you bring to the table

Degree in social work (Bachelor's, Master's or Diploma) or a comparable qualification as well as professional experience in the field of hospital social work or in related health care disciplines
Comprehensive knowledge of social law, especially in the areas of SGB V, SGB XI and SGB XII
Strong social skills, strong communication skills and empathy in dealing with patients and their relatives
Confident handling of IT applications such as MS Office, Windows operating systems and SAP
Excellent time management, independent and solution-oriented way of working as well as a strong sense of responsibility
You have the legally required measles immunity certificate
